Required forms include:
- Pre-Participation Physical Evaluation
- Concussion Forms and Gfeller-Waller Information
The Pre-Participation Physical Evaluation and Concussion Forms can be found on the North Carolina High School Athletic Association (NCHSAA) website under the Health and Safety tab. Scroll down to the For Students & Parents section to find the forms.
- Insurance Form (needs to be updated)
All required forms/paperwork are filled out online in FinalForms by families and students. Click the link for instructions on how to create an account and use - FINAL FORMS.
Physical Evaluation
Students must have a physical evaluation completed by a healthcare provider. The Wilson Area School-Based Health Centers (WASH) are an option for interested families. The WASH Centers provide accessible and affordable physical and preventive health services to students and staff. WASH is located at Darden Middle, Forest Hills Middle, Beddingfield High, Fike High and Hunt High Schools. All Wilson County Schools students and staff members can be seen by appointment at WASH. WASH bills private insurance, Medicaid, Medicare and has a sliding scale fee for non-insured. Same-day appointments available. Please call to schedule an appointment or email WASH at wash@wilsonschoolsnc.net. There are forms that need to be completed to be seen at WASH. Access those forms from the Wilson County Health Department website.
