Medication Authorization Form

Click the link to access the Medication Authorization Form. 

*To type on the form, follow these steps (though it might be best to print it and complete it by hand to preserve the formatting):
1. Click on the link to the Medication Authorization Form
2. When it opens as a Google Doc, click File in the upper left of the screen
3. Select Make a copy

*To translate the form, follow these steps:
1. Click on the link to the Medication Authorization Form
2. When it opens as a Google Doc, click Tools in the upper left of the screen
3. Select Translate document and choose the language
