Student Survey Information

Wilson County Schools students in grades 3-12 will be taking several surveys during the 2025-2026 school year. The surveys are listed below. Click the links to view the survey questions. If you would like to opt out your student(s) from taking surveys, you can do so by clicking the link below to the Student Survey Opt Out Form. If you opted your student from the survey in the Fall, you do not need to complete the opt out form for the Spring.

Opt Out Form

If you would like to opt out of any/all surveys, please complete the form below. To translate the form into your language, right click on the form, click Translate and select your language. The Opt Out Form can be completed for the Fall surveys from Sept. 1-19, 2025. The Opt Out Form can be completed for the Spring surveys from Feb. 1-13, 2026. The Opt Out From can be completed for the Spring surveys from Jan. 16-Feb. 13, 2026.

2025-2026 Panorama Surveys
Fall (October 6-16, 2025 window) Spring (February 23-March 6, 2026 window)
Opt Out deadline is September 19, 2025 for the Fall surveys and February 13, 2026 for the Spring survey. If you choose to opt out after the deadline, you will need to write a letter to your student’s principal.

  To translate the Google Doc, follow these steps:

2025-2026 Student Perception Surveys
January (January 26-30, 2026 window)
Opt Out deadline is January 16, 2026. If you choose to opt out after the deadline, you will need to write a letter to your student’s principal.

2025-2026 Pride Survey
March (March 2026)
This is an Opt In survey. Opt In Forms will be sent home with students to be signed by parents/guardians and returned to school.
