Medication and Required Immunizations Information

If your student will take medication at school this year, an updated Medication Authorization Form is required. School nurses sent forms home with students who received medication at school last year. If you need a new form for your student’s healthcare provider to complete, please contact your student’s school.

This is also a reminder about state immunization requirements. Kindergartners, 7th graders, 12th graders, students attending an NC public school for the first time, and Wilson Academy of Virtual Education students must provide proof of the required immunizations unless they have a valid exemption. These students must provide immunization records within 30 calendar days of the first day of school. The first day of school for traditional schools is August 24, and the first day for Early Colleges is August 5. State law requires schools to exclude students after the 30th day if the required documentation has not been received. Students may return once proof of immunization is provided.

All kindergartners and students attending an NC public school for the first time must also submit a health assessment completed by a medical provider. Health assessments completed within one year before the first day of school will be accepted.

If your child needs immunizations or a health assessment, please schedule an appointment with your healthcare provider as soon as possible.
