The WASH Clinic and school nurses are hoping to get current 11^th graders vaccinated before school gets out for summer. The registration form will come directly to WASH.  The QR code asks for their child’s name, parent’s name, email & phone number.  Once they submit the form an email will be sent to the parent containing the letter, registration form, and consent for vaccines. Upon submission, WASH will be notified and will contact parents.
